UTR 2. National Insurance number 3. Bank account number and sort code you want HMRC to pay the grant into 4. Government Gateway user ID and password - if you do not have a user ID, you can create one by clicking on this link:https://www.access.service.gov.uk/registration/email *Check if you’re eligible to claim* You can use an online tool to find out if you’re eligible to make a claim – please follow this link: https://www.tax.service.gov.uk/self-employment-support/enter-unique-taxpayer-reference According to current guidance, accountants are not allowed to make the claim for you using existing authority (it may change later).
